The KMDA invites you to attend an exciting engagement at The Thrive Center on Wednesday, December 16th.
We want to discuss hospital/post-acute relationships, identify current challenges which impede best practice models, and consider strategies to improve our patient’s experiences and outcomes.
Our three initial objectives are: 1) Transitions of Care: i.e. patient handoffs (from hospital to post-acute, and post-acute to home with focus on preventing re-hospitalizations). 2) Advance Care Planning and goals of care through the implementation of the MOST form. 3) Proactive, meaningful systems for provider communication across settings of care.As our meeting concludes, we will summarize potential, practical strategies as well as logistics for moving the collaborative efforts forward.
